Midwifery Questionnaire:

  • Are you drug and alcohol free?

  • Are you aware we do not take insurance?

  • Are any severe health issues you have had currently resolved?

  • Are you willing to work on the body as a whole, including spiritual, mental, and physical aspects?

  • Do you want to have your baby in the comfort of your own home?

  • Are you willing to invest into your pregnancy (including research) and work to make a truly informed decision in your care?

  • Do you want to lead your care with guidance from a trained and experienced Midwife?

  • Do you want care from a solo-practice midwife?

  • Are you comfortable with this including the need to cancel/reschedule appointments with short notice due to other labors/needs?

  • Do you acknowledge that with home appointments comes a grace period for the midwife to arrive?
